What forms do I need to apply for my Assureflex HSA?
Claims
- Claims are processed in 1 to 2 business days (except year-end and Jan. 1st. to 15th.)
- Claims are paid by Cheque or by Direct Deposit, as selected on the Enrollment Form
- Claims payments are tax free to the Employee (except Provincial tax in Quebec)
Invoices
- Invoices are emailed to Employers on the 1st. or 15th., as selected on the Application
- Invoices are paid by Cheque, e-Transfer or EFT, as selected on the Application Form
- Invoices consist of Claims plus Administration plus applicable Ontario taxes
